#OnThisDay September 16, 2014: Fatal Underground

A fatal industrial accident at the West Ridge Mine in Carbon County, Utah, marked a serious episode in the modern history of coal mining in the Book Cliffs. At approximately 1:39 a.m., 46-year-old Alejandro Ramirez of Price was fatally injured at the entrance to the mine’s 23rd East Longwall section. Ramirez was operating a Wagner ST-2D mobile diesel can setter when the articulated machine crushed him. The Utah Labor Commission recorded that there were no witnesses to the accident. Contemporary reporting identified Ramirez as an employee with approximately six years of service at the operation.

The accident occurred at an important underground coal operation within Carbon County’s long-established mining economy. West Ridge Mine was located near East Carbon and Sunnyside in the Book Cliffs coalfield. The Utah Geological Survey records that West Ridge began operation in 1999 and extracted coal from the Lower Sunnyside bed. Longwall mining formed the central production method. In this system, mechanized equipment removes coal along an extended underground face while powered supports protect the active working area. As extraction proceeds, the equipment retreats through the panel. Federal mine-safety proceedings concerning West Ridge describe the mine as operating under deep cover and document the geological pressures associated with underground mining in the area.

By 2014, West Ridge had become a substantial contributor to Utah coal production. Federal mining data show that the mine produced approximately 2.5 million short tons of coal that year and employed more than 200 workers. Production during the preceding years had also reached several million tons annually. These figures placed the September accident within a large mechanized operation rather than a small or marginal mining enterprise. West Ridge formed part of a broader Carbon County industry that had shaped settlement, employment, transportation, and community development in eastern Utah for generations.

The machine involved in Ramirez’s death performed a specialized support function associated with underground mining. According to the Utah Labor Commission, the Wagner ST-2D mobile diesel can setter articulated while Ramirez was operating it, crushing him. The accident occurred specifically at the entrance to the 23rd East Longwall section. The state’s official description is significant because it distinguishes the event from a roof collapse, explosion, fire, or methane incident, hazards that have historically figured prominently in Utah coal-mining disasters. Initial public reports contained uncertainty about whether a cave-in had occurred, but the official state record subsequently identified the equipment movement as the fatal mechanism.

The accident also entered Utah’s formal mine-safety record under the state Coal Mine Safety Act. The Utah Labor Commission’s Office of Coal Mine Safety was required to compile major mine accidents and emergencies and report them to the governor and legislature. The 2014 safety report included Ramirez’s death among the year’s significant coal-mine incidents. During that period, the Office of Coal Mine Safety visited operating mines, participated in mine-rescue activities, worked with emergency-planning organizations in Carbon and Emery counties, and coordinated with federal agencies and industry representatives. The state report therefore preserved the West Ridge fatality within a larger system of mine-safety oversight that had developed in response to the continuing hazards of Utah underground coal production.

The September 16 fatality was especially notable in the operating history of West Ridge. Contemporary reports stated that it was the mine’s first fatal accident during approximately fifteen years of operation. Federal Mine Safety and Health Administration data from the period also document injuries, inspections, citations, and enforcement activity at West Ridge, illustrating the regulatory environment surrounding a major underground coal mine. Ramirez’s death occurred seven years after the 2007 Crandall Canyon Mine disaster elsewhere in Carbon County, an event that had intensified Utah attention to underground coal-mine safety and emergency preparedness. The West Ridge accident was different in scale and cause, but it demonstrated that individual equipment-related hazards remained capable of producing fatal consequences even in highly mechanized mines.

West Ridge itself was approaching the end of its productive life. The Utah Geological Survey later reported that UtahAmerican depleted the remaining recoverable coal under lease and shut down production in late November 2015. Across roughly seventeen years of operation, the mine produced about 43.7 million short tons of coal. Ramirez’s death therefore occurred during the final period of active production at one of Carbon County’s significant modern underground mines. Remembered within Utah’s official safety records, the September 16, 2014 accident documents both the human cost of coal extraction and the continuing importance of equipment safety, regulatory inspection, and accident investigation in Utah’s modern mining history.
